Male fertility & sperm health · Stockton Heath

“Normal” sperm results
are not the same as optimal.

The “normal” cut offs on a semen analysis are set at the lowest 5% of results from men whose partners got pregnant within a year. So normal means his results cleared that minimum. It does not mean they are where they could be.That part rarely gets explained.

Semen analysis read against optimal ranges, DNA fragmentation, his own bloodwork, acupuncture and nutrition. In person in Stockton Heath, or online.

Available in person or online, wherever you both are.

Before any of the practical bits

You are probably not the person this page is about.

You are probably the one reading it at 11pm with the laptop open on the kitchen table, working out whether to send it to him. And weighing up, as you read, whether it will land as here is something useful or as I think this is your fault.

So we wrote it to be sent.

And if you’re him

Nothing on this page says this is your fault. Because it isn’t.

You were handed 1 test and 1 word. The word was doing a job it was never built to do, and there probably wasn’t time to sit you down and explain what it actually meant. That is not a failing on your part. Male fertility often gets a few minutes at the end of an appointment that was mainly about your partner.

This page is not a diagnosis and it is not an accusation. It is the explanation you should have been given, and a list of things worth knowing.

The bit the test doesn’t look at

A semen analysis looks at the outside of the sperm.

How many there are. How well they swim. What shape they are. All of that is the outside.

It does not look at what is inside.

And what is inside is half the blueprint for your baby. The genetic instructions, carried in that sperm, making up 50% of everything that comes next.

A sperm can be present in good numbers, swim beautifully, be exactly the right shape, and still carry damage that no microscope looking at its outline will ever pick up. Which means a perfectly reassuring report and an unanswered question can sit side by side, for years.

DNA fragmentation

In our view, this should be a baseline test.

That is our opinion and we will say so plainly rather than dress it up. It is not routinely offered on the NHS, which is why most couples have never heard of it, and why we are telling you here rather than assuming somebody already did.

It matters most if you have been at this a while. If everything on the outside looks fine and nothing is happening, then the question of what is inside has never actually been asked.

But there are different types of DNA fragmentation test, and which one matters. That is exactly the kind of thing we help you navigate inside our work together during a Both on Board session, should we feel it is necessary for you.

Ask us about it · free 15 min call

We are not doctors and we do not diagnose. We will not tell you what a fragmentation result means for your chances, because it cannot be predicted, and anyone who quotes you a number should be asked for the evidence. What we will do is make sure the question gets asked at all.

Questions people ask

Depends what you want that word to mean. Normal means his numbers sit above the lowest 5% of men whose partners had already conceived within a year. It is the floor of the room, not a target, and the World Health Organisation say in their own manual that the figure is not meant to separate fertile men from infertile ones. So if you have been trying a long time and the only thing standing between you and an answer is that one word, yes, it is worth a second look.

Count, motility, morphology, pH and round cells, read against optimal ranges for fertility rather than against the lower reference limit. Some of those you will have skimmed past on the page. Some you will not have noticed were there. We go through each one and tell you what it means, which is usually the first time anyone has read that report out loud.

No. We review the one you already have, or tell you how and where to get one and what to ask for. Anything you send us needs to be dated within the last 12 weeks, because sperm made 6 months ago is not the sperm you have now.

A standard semen analysis looks at the outside of the sperm: how many, how they swim, what shape they are. DNA fragmentation looks at what is inside, which is the genetic material making up half the blueprint for your baby. A sperm can look completely fine from the outside and still carry damage that no microscope reading its outline would pick up. In our view it should be a baseline test. It is not routinely offered on the NHS. There are also different types of DNA fragmentation test, and which one matters, so it is exactly the kind of thing we help you navigate inside a Both on Board session, should we feel it is necessary for you.

We are not going to tell you that, because it cannot be predicted, and anyone who quotes you a number should be asked for the evidence. What we can tell you is what this actually is: someone reading his results properly, looking at his bloods, and going through the things across a 90 day window that are genuinely in his hands. That is not a promise. It is considerably more than 1 test and 1 word.

Because sperm takes roughly 70 to 90 days to be made, which means the sample he gave in March is a report on January. The flu, the work trip, the fortnight of no sleep. It also means that in 3 months it is an entirely different set of sperm. This is the one part of the whole process where you can do the 12 weeks and then look again, because you can retest sperm. You cannot retest an egg.

Come together. It takes 2 of you.
